Referrals - Privacy Notice

The purpose for this processing

To make referrals (with consent of those being referred or those legally acting on their behalf) to relevant local and locally operating services.

Legal basis for this processing

Article 6 Lawfulness of Processing (Section 1, (a)):

The data subject has given consent to the processing of his or her personal data for one or more specific purposes.

What data do we collect about you

  • Name
  • Address
  • E-mail address
  • Telephone number
  • Date of Birth

How long do we keep your personal data

Within the Referral System – Personal, identifiable data is only retained for a maximum of 90 days before it is disposed of.

Who we may share it with

  • Relevant internal officers and departments
  • External third-party agencies on the referral system eg. voluntary partners such as Citizens Advice, Summit and CVST, statutory partners such as the Police, Essex Fire, Health partners including GPs, ICB and other Local Authorities
  • Family members/those acting on behalf of data subject

Where we get it from

  • The data subject themselves
  • Family members/those acting on behalf of the data subject
  • Next of kin
  • Health care professionals (e.g. GP)
  • Other referral agencies
